Insurance Information
Insurance companies vary widely in their coverage for neuropsychological and psychological evaluations. We advise all of our clients to check extensively with their insurance company about benefits prior to the evaluation in order to avoid unexpected out-of-pocket expenses. Please note that your health insurance cannot be utilized neither for legal evaluations nor for testing that is solely for academic purposes (e.g., academic accommodations, independent school admissions, etc).
If we do not accept your insurance, you will be provided with a “superbill” that has all the information your insurance company needs to process your claim, which you will need to submit to your insurance company to seek reimbursement directly to you.
The patient will be responsible for deductibles and/or co-pays, which will be collected at the time services are rendered unless other arrangements have been made.
Referrals
Always begin by calling your insurance company to confirm whether your plan requires a referral from your physician or another referring provider. Every insurance plan is different, and some require referrals while most do not.
If your insurance does require a referral:
Please contact your referring doctor and ask them to fax the referral to us at 617-431-8968 before your appointment can be scheduled.If your insurance does not require a referral:
We still strongly recommend that the provider who is referring you sends us a detailed clinical note explaining the reason for the referral. This note should be faxed to 617-431-8968 to help us understand the concerns being evaluated and ensures that we select the most appropriate assessment or therapeutic approach for your needs.
Payment Quick Facts
Co-pays: Your insurance company requires our office to collect co-pays at the time of service, so please be prepared to pay at the time of your telehealth intake or bring the means of payment with you for on-campus visits.
Self Pay Patients: You are required to pay in full at the time of your visit, unless arrangements have been made in advance with our office manager.
No Show Policy: We charge a fee of $80 for missed appointments, payable prior to or on your next appointment.
Late Cancellations: Appointments cancelled less than 48 hours prior to scheduled time, will be billed a late cancellation fee of $80
Method of Payment: We accept cash, personal checks and all major credit cards. Please note that if your check is returned to us, a $35 service charge will be added to your balance and will need to be paid in full within 10 days.
Copies of Medical Records: We will send your medical records to another provider at no cost to you.
Fees for completion of forms: If our providers are asked to complete a medical or school related form without an office visit (insurance policies, physical forms, disability applications, etc.) there will be a charge of $10 per page. This fee must be paid before forms can be picked up or submitted.
Past due accounts: We will make every effort to work with you on payment plans, although we do not carry balances over 2 years. Any cost incurred in trying to collect payment will be added to your account. Failure to make payments will result in your account being sent to a collections agency and will result in the termination of the provider/patient relationship.
